Creating an Addendum
You can add supplementary information to a final, signed report by creating an addendum.
Adding an addendum does not lock the order. It is still available to other users who would like to add an addendum.
After an addendum is transcribed, it appears in the To Sign tab in the Signing worklist, where you can review, correct, approve, and sign the addendum.
When you transcribe an addendum for linked orders, a separate entry appears for each linked order in the Signing worklist. You must sign each addendum separately.
To create an addendum:

From the worklist, click the Dictated tab.
The Dictated tab lists the orders that have been dictated.

Do one of the following:
- Double-click the order.
- Right-click the order and choose View.
- Select the order and then press Enter.
The Multi-Method Reporting window appears.
If you opened a linked order, InteleViewer opens and loads the images of all the linked orders. Also, the Patient Record tab displays the report and audio recording for all the linked orders.
- Select an addendum template from the Templates menu.

Dictate the addendum by doing one of the following:
- Use the dictation controls.
- Use the digital voice recorder controls.

When you have finished recording your dictation, do one of the following:

Edit Report
Display this draft addendum in the Report Editor with the sections and text from the specified report template. Multi-Method Reporting switches to self-editing mode, allowing you to edit your draft addendum manually or with voice commands. See Self-Editing the Draft Report.
The Edit Report button is displayed only when you are using MMR with M*Modal in Live mode. See Understanding Live Mode and Self-Editing Mode.
If M*Modal becomes unavailable, you are given the option to send the draft addendum to the transcriptionist.
Save Draft
Save your audio recording as a draft addendum. The audio recording is associated with the addendum. If you open an order that has a saved draft addendum, the audio recording is opened.
If you are dictating the addendum for linked orders, InteleViewer associates the same audio recording to each linked order. See Signing a Report or Addendum.
You can resume only your own saved drafts. You cannot edit drafts saved by other users.
Send Addendum to Transcriptionist
Bypass M*Modal processing and send your audio recording to a transcriptionist for transcription. If you are sending an audio recording for linked orders to a transcriptionist, the same audio recording is sent for each linked order.
You can also re-open and edit the addendum before the transcriptionist has opened it. See Editing Sent Dictations or Addendum Dictations.
Sign Addendum
Bypass the transcriptionist and sign the addendum. The addendum will be removed from your worklist.
If your site is configured to use the Resident Over-Reads module, and you are a resident radiologist, you must select a reviewing radiologist to enable the Sign button.
If you are fellow radiologist, you must select an attending radiologist to enable the Sign Addendum button.
